    SD27J NUTRITION SERVICES CIVIL RIGHTS COMPLAINT PROCEDURE
     
    Policy:  United States Department of Agriculture (USDA)/Food and Nutrition Service (FNS) Instruction 113-1 (dated 11/8/05) delineates the civil rights requirements for participants in Child Nutrition Programs (CNP.)  The following is required at the local school food authority (SFA) level.
     
    Any person alleging discrimination based on race, color, national origin, sex, age, or disability has a right to file a complaint within 180 days of the alleged discriminatory action.  All civil rights complaints within School District 27J's Food and Nutrition Services Department should be directed to Tony Jorstad.  Tony Jorstad can be reached via phone at 303-655-2985 or by email at tjorstad@sd27j.net.  Complaints can be submitted verbally, in writing, or anonymously.
     
    School District 27J's Food and Nutrition Services Department will obtain all necessary complaint information and process the complaint within 90 days.  The complaint will be forwarded to the Colorado Department of Education Nutrition Unit.
     

    PROCEDIMIENTOS DE QUEJAS DE LOS DERECHOS CIVILES DE LOS SERVICIOS DE NUTRICIÓN DEL SD27J

    Reglamento:  Departamento de Agricultura de los EE.UU. (USDA)/Servicios de Comida y Nutrición (FNS) Instrucciones 113-1 (fecha 11/8/05) describe los requisitos de los derechos civiles de los participantes en los Programas de Nutrición de Menores (CNP.)  Lo siguiente es obligatorio a nivel de la autoridad local de alimentos escolares (SFA).

    Cualquier persona que alegue discriminación basado en la raza, color, origen nacional, sexo, edad o discapacidad tiene el derecho de presentar una queja dentro de los 180 días después de la supuesta acción de discriminación.  Todas las quejas civiles dentro del Departamento de Servicios de Nutrición del Distrito Escolar deben ser dirigidas a Tony Jorstad.  Tony Jorstad puede ser contactado por teléfono al 303-655-2985 o por correo electrónico a tjorstad@sd27j.net.  Las quejas se pueden presentar verbalmente, por escrito o anónimamente.

The letter must contain the complainant’s name, address, telephone number, and a written description of the alleged discriminatory action in sufficient detail to inform the Assistant Secretary for Civil Rights (ASCR) about the nature and date of an alleged civil rights violation. The completed AD-3027 form or letter must be submitted to USDA by:
