Vendor Setup Form

Welcome to the 27J Schools Vendor Setup Form

To stay compliant with 27J Schools policies, vendors are required to complete this form every other year or whenever there is a change of address.

Please be prepared to:

  • Provide basic vendor information

  • Upload a current W-9 (Request for Taxpayer Identification Number and Certification)

  • Indicate whether you are a PERA retiree

Why We Ask About PERA Retiree Status
Colorado law requires PERA employers, including 27J Schools, to report compensation paid to PERA retirees, even if services are performed through a business. Retirees are subject to a 110-day per calendar year limit. If you receive PERA benefits, payments from 27J Schools may impact your PERA status and must be reported.
