• REQUIREMENTS TO PARTICIPATE IN ATHLETICS

     

    1. The office must have a current physical on file.
     
    2. Completed and turned in OTMS Athletic Participation Form 2017-2018

    3. The $75.00 (per sport) fee must be paid prior to participation OR a completed 2017-2018 School Fees Waiver Form, if the student receives Free/Reduced lunch.

    4. The $25.00 fundraising fee (this fee cannot be exempted/waived). 

    5. All outstanding SD27Jfees must be paid prior to participation in the sport.

     

    Athletes will not be added to the roster until all of the requirements above are met.

    • 2017-2018 Permission form completed and turned in
    • $100.00 Fee Paid OR 2017-2018 School Waiver Form completed and $25.00 Fee Paid
    • ALL Outstanding Fees Paid
