REQUIREMENTS TO PARTICIPATE IN ATHLETICS1. The office must have a current physical on file.2. Completed and turned in OTMS Athletic Participation Form 2016-2017
3. The $75.00 (per sport) fee must be paid prior to participation OR a completed 2016-2017 School Fees Waiver Form, if the student receives Free/Reduced lunch.
4. The $25.00 fundraising fee (this fee cannot be exempted/waived). If your student would like to participate in an actual fundraiser, please see Mrs. Key for further details.
5. All outstanding SD27Jfees must be paid prior to participation in the sport.
Athletes will not be added to the roster until all of the requirements above are met.
