Under penalty of perjury, we affirm that all information given above is true and correct. This is the home where the student will regularly eat and sleep while attending District 27J schools. It is understood that all school records will contain this address and that all communications sent by the school will be to this address. District 27J may qualify student(s) for enrollment based on this Co-Resident Enrollment form and in accordance with the district policy or state and/or federal law pertaining to students and families lacking traditional proof-of-residence verification. We understand and agree that if it is later determined that one or more students enrolled with this Co-Resident Enrollment form are not legal residents of the boundary school(s), such student(s) will be withdrawn immediately from said school(s). We further agree to pay District 27J schools any and all applicable tuition charges, which may be due, together with the cost of collection thereof, including reasonable attorney’s fees. All signatures below are necessary to complete the enrollment process.
